What the bands describe, and the difference between association and prediction
The adult BMI categories are boundaries drawn on a continuous scale. There is nothing physiologically special about 24.9 becoming 25.0. The bands exist because population data showed risk rising across certain ranges, and public health needs thresholds it can act on.

Across large populations, higher BMI is associated with increased likelihood of type 2 diabetes, high blood pressure, cardiovascular disease, certain cancers, obstructive sleep apnoea, gallbladder disease and osteoarthritis of weight-bearing joints. The strength of those associations generally increases across the obesity classes.
The word doing the work in that paragraph is associated. These are patterns observed across many thousands of people. They describe how likelihood shifts across a population, and they are genuinely useful for that. They do not tell an individual what will happen to them.
Underweight gets far less attention and carries real risk of its own: nutritional deficiency, reduced bone density, impaired immune function, and complications around fertility and pregnancy. A low BMI can also be a signal of an underlying condition or of disordered eating, both of which need a provider rather than a calculator.
The standard cut-offs were derived largely from European-ancestry populations. The World Health Organization has noted that the relationship between BMI, body fat and health risk differs across populations, and that for some Asian populations risk begins to rise at lower BMI values than the standard thresholds suggest. Several countries apply lower cut-offs for this reason.
Age matters too. Body composition shifts across the lifespan, and the BMI range associated with the best outcomes in older adults is not identical to that in younger ones. A number that is stable across decades does not mean a body that is.
BMI is a screening tool. Its job is to sort a population into groups worth a closer look and groups less likely to need one. It is very good at that and very cheap. It was never designed to tell one person about their own health, and it cannot.
